Repeat prescriptions can either be requested:

All repeat prescription requests that arrive on the surgery premises before 10.00am will be available within 24 hours Monday to Friday.

Note that for requests received on a Saturday, collection day will be the following Monday after 2.00pm.

You will be given a computerised repeat prescription request slip (counterfoil) which allows you to tick the items you require when ordering. If you do not have a counterfoil for re-ordering purposes, please put your repeat prescription request in writing.

No repeat prescription requests will be taken over the telephone for medico-legal reasons.

Prescription Forms

Here are two main parts to the printed prescription form that you will receive from us.

The left hand side details the medication you have been prescribed and need to collect from a chemist.

note: the "notes overleaf" referred to at the bottom relate to whether or not you have to pay for your prescriptions.

The right hand side is for you to tear off and keep until you are running low on your prescribed medication.

It is then to be completed by ticking:

  1. which medication you require
  2. where you want to collect it

Finally return to the surgery and we'll arrange for your medication to be available in your preferred location after 24 hours.

It also indicates when a medication review is required. You may not be prescribed medication after this date until the review has been completed.

Prescriptions

A prescription for medication may be issued following a consultation with a doctor.

A form will be issued detailing what is to be taken and when.

The prescription form can be taken to a chemist for the medication to be dispensed, or surgery dispensary if you qualify for this service, please check with surgery by phone or on your next visit.

A repeat prescription may be issued up to the recommended review date. After which point a medication review may be necessary.
